Submit Your Application
Once you meet the eligibility requirements, contact us. We’ll help you complete your application and send it off for NDIS approval.
Create Your Plan
Once you’re approved. We’ll work with you, your family, and/or an advocate to create a personalised service and care plan. Your plan will be designed to assist you in maintaining and improving your lifestyle, whilst increasing your independence and involvement in the community.
Adjust Your Plan
We’ll get to know you, your strengths, and your preferences so we can adjust your tailored plan as you grow and your circumstances change. It’s all a part of the flexible support you’ll receive throughout your Advocate Care journey.
